Effective care Our goal is to provide each patient with the information, education and tools to experience their best health possible. One way we measure that is by making sure our patients do not require more assistance after being discharged. We are happy to report that our efforts toward effective patient care have created a steady decline in patients returning to the hospital for more services. A safe place The safety of our patients and staff is also an area that we strive to excel in, and we are extremely proud to say we have received an “A” safety grade from Leapfrog for the fourth year in a row. This national distinction is a testament to our commitment to prioritizing patient safety. A report on our quality and safety WINDWARD HEALTH | 3 Patient satisfaction Adventist Health Castle participates in the Hospital Consumer Assessment of Healthcare Providers and Systems survey, known as HCAHPS, which measures patients’ perspectives on hospital care. This standardized tool allows objective and meaningful comparisons between hospitals in areas that are important to consumers. Because it shows the level of trust and confidence our patients have in us, one of the scores that means the most to us is, “Would you recommend this hospital to your family and friends?” We are honored that for four years in a row, you have ranked us in the top 25% of hospitals nationwide on this question. 2018 2019 2020 2021 80th PERCENTILE 85th PERCENTILE 82nd PERCENTILE 84th PERCENTILE Top 25% in the nation

Your safety is our priority, and we work diligently to prevent harm such as falls, infections and injuries to your skin. We were recognized by The Leapfrog Group with an “A” letter grade in 2020 and 2021. Reducing readmissions At Adventist Health Castle, our goal is to provide each patient with the information, education and tools they need to experience their best health possible. One way we measure our success is by closely monitoring our rate of patients returning to the hospital within 30 days of an inpatient admission. Over the past five years, we have seen a steadily declining readmission rate year over year. How did we do this? We have implemented best practices such as: • Bedside reports when patient care is handed off from one nurse to the next. • Care planning sessions with all departments involved in your care. • Doctors and nurses discussing your care with you daily. • Detailed reviews of your home medication list upon admission. • Discharge phone calls by a specially trained care transition nurse. FACE F is for Face: Does the person’s face look uneven? ARMS A is for Arm: Is one arm hanging down? SPEECH S is for Speech: Is the person’s speech slurred? Does the person have trouble speaking or seem confused? TIME T is for Time: Call 911 now! Our stroke program achieved Gold Plus recognition from the American Heart Association’s Get With The Guidelines program in both 2020 and 2021. We have also been certified by The Joint Commission as a Primary Stroke Center since 2018. These achievements mean that we consistently follow the latest scientific treatment guidelines that have been shown to improve patient outcomes. Learn the signs and symptoms of stroke Stroke care Strokes are a leading cause of death and disability in the U.S. Every year, more than 795,000 adults have a stroke. Strokes are both preventable and treatable. Patients with signs and symptoms of stroke should call 911 and seek medical care immediately for early evaluation, diagnosis and treatment. Signs and symptoms of a stroke can easily be identified by the BE FAST acronym (below). Certified Primary Stroke Center

CDC recommends an assessment by a healthcare professional if you have one or more of these signs or symptoms. Act fast! Sepsis is a medical emergency. If you or your loved one has an infection that is not getting better or is getting worse, seek medical attention right away. Adventist Health Castle is committed to the best care possible for sepsis patients. This begins with early recognition of sepsis and quick treatment with diagnostic lab tests, IV antibiotics and IV fluids. The Centers for Medicare & Medicaid Services has a best practice bundle for sepsis care. Castle’s performance on bundle compliance, at 90%, is well above the national (57%) and state (73%) averages. Source: Centers for Medicare & Medicaid Services Emergency care Adventist Health Castle’s Emergency Department (ED) is staffed by board-certified physicians and specialists 24 hours a day. If you are experiencing a medical emergency, a fast and accurate diagnosis is the key to determine what steps need to be taken for your best outcome. Our team is dedicated to providing efficient and compassionate care when you need it most. One key measurement we track is the time it takes for our patients to see an ED physician. The national average is 30 minutes, with the best ED wait time at 10 minutes. If you or a loved one is experiencing a life-threatening medical emergency, please call 911. Sepsis care What is sepsis? According to the Centers for Disease Control and Prevention (CDC), sepsis is the body’s extreme response to an infection and is a life-threatening emergency. This happens when an infection you already have triggers a chain reaction throughout your body. Risk factors for sepsis are: • Age—being 65 years or older or younger than 1 year. • Having a weakened immune system. • Having chronic medical conditions, such as diabetes, lung disease, kidney disease or cancer. • Having a recent severe illness or hospitalization. • Having had sepsis before.
