Reducing readmissions At Adventist Health Castle, our goal is to provide each patient with the information, education and tools they need to experience their best health possible. One way we measure our success is by closely monitoring our rate of patients returning to the hospital within 30 days of an inpatient admission. Over the past five years, we have seen a steadily declining readmission rate year over year. How did we do this? We have implemented best practices such as: • Bedside reports when patient care is handed off from one nurse to the next. • Care planning sessions with all departments involved in your care. • Doctors and nurses discussing your care with you daily. • Detailed reviews of your home medication list upon admission. • Discharge phone calls by a specially trained care transition nurse.
